Job Title: Client Care Navigator

Term: Full-Time

Location: Moncton, NB or Beresford, NB

Summary:

This role combines Client Care Navigation (CCN) with adjustment to vision loss services for clients with a vision loss. The CCN work involves being the key contact  for individuals after a vision loss diagnosis. Responsibilities include assessment, supporting the client to achieve their care plan goals, and strengthening the communication and linkages between VLR Staff and external community health care providers across the continuum of care.

The emotional support side of the role requires the individual to demonstrate the professional ethics and practices expected of a counsellor as well as being able to offer an adjustment to vision loss service that is responsive, confidential, person centred and focussed on adjusting to vision loss.

This role strives to ensure clients achieve the following outcomes:

  • To provide individuals with vision loss, healthcare navigation to increase and enhance the healthcare system's capacity.
  • To improve the client's experience and health.
  • To enhance a client's capacity to remain independent for as long as possible.

Your impact at VLRC:

  • Responsible for the completion of the registration, intake assessment, service coordination and review process for Vision Loss Rehabilitation.
  • Act as a main point of contact for care navigation and support.
  • Seeks feedback on client satisfaction and take remedial action as required.
  • Provide information, advice and guidance to ensure that people continue to be supported throughout their sight loss journey.
  • Provide a high quality, professional and confidential emotional support service to people with vision loss in individual and groups settings.
  • Provide an initial assessment and associated administration to establish interventions that would be helpful to the client in their adjustment to vision loss.
  • Develop a structured and agreed set of sessions that will initially meet the needs of the client(s) followed by a formal re-assessment and review of these plans with the client at a predetermined time. 
  • Utilise the most appropriate methods to meet the clients’ individual needs and assist with achieving their outcomes.  Adapting this intervention style as needs or presentations change.
  •  Deliver sessions virtually either by telephone or over web-based systems or in a clients preferred location.

General Responsibilities:

  • Complete documentation that reaches the required professional standard of a provider of counselling and meets the organisational client health care record reporting requirements.
  • Work in a model that encourages individual independence. but acknowledges at times, support is a necessary part of achieving this outcome and prepares clients for this transition.
  • Deliver service in a variety of locations, including the clients’ homes, places of work, local community as needed
  • Encourage clients to advocate for themselves if they have the ability to do so.
  • Support client by advocating on their behalf on matters relating to accessing services.
  • Provide professional and appropriate referrals to other internal and external programmes/ teams that will benefit the client.
  • Manage a caseload that enables a maintenance of competence to practice and the opportunity for peer review and appropriate supervision provision.
  • Organize and prioritize client referrals to facilitate a relevant and responsive service.
  • Work within VLR policies, procedures, practices and timeframes
  • Speaks and writes clearly, concisely and actively shares information and ideas and encourages others to share their views and concerns.
  • Full participation in approved mandatory, organisational and professional practice training and performance management.
  • Open to learning and proficient in the use of technology to be efficient and effective in their everyday work.
  • Seek opportunities to build and maintain community agency relationships and increase the awareness of the specialised services that Vision Loss Rehabilitation offer.
  • Adhere to COVID-19 policies and procedures to ensure the safety of themselves, colleagues and clients.

Relationships

  • Builds and sustains effective therapeutic relationships with clients.
  • Interacts with clients, their family, and caregivers to develop and identify vision related goals.
  • Builds and maintains good working relationships with professionals in other agencies and community groups.
  • Work collaboratively with other agencies to ensure relationships and client needs are communicated and understood within ethical boundaries.
  • Participates in inter-disciplinary team meetings.
  • Understanding of roles and linkages between the Primary and Specialized Service Provider with the Service Delivery Model.
  • Manage a caseload that enables maintenance of competence to practice and the opportunity for peer review and appropriate supervision provision.
  • Work within the preference that where possible clients are provided services virtually, in the Vision Loss Rehabilitation NB’s determined venues but where prudent services are delivered in a way that meets the client needs most appropriately.

Who are you?

  • You have client Service experience in health and/or social service sectors.
  • You have proven experience of delivering person centred emotional support services.
  • You have expertise in group facilitation skills.
  • You have counselling skill and professional conduct that meets practice standards or the equivalent.
  • You have knowledge of client centred philosophy and interviewing, assessments, and planning practices.
  • Have excellent communication skills (verbal and written).
  • Have effective case management skills.
  • You are proficient use of the Microsoft Office suite of programmes.
  • You are able to provide a comprehensive assessment of an individual’s needs in a variety of settings and to develop an appropriate programme of counselling.
  • You are bilingual (English/French).

  • Registered health professional designation is an asset.
  • Post-secondary qualification in Social Work, Counselling or other related health care discipline.
  • 3 or more years’ experience working with individuals with seniors and/or people with a disability preferably vision loss.

  • Willingness to work flexible hours.
  • Ability to work remotely and in the office.
