Supporting Independence Through Vision Rehabilitation 

If any of your patients have experienced blindness or significant vision loss, we are here to provide the necessary skills and training for them to lead independent, active lives. 

As a valued allied health professional (such as an occupational therapist, nurse, physical therapist, social worker, or discharge coordinator), you can make referrals to our service. This will enable our experienced staff to develop a tailored rehabilitation plan for each person.

If you're a health care professional who knows someone who's experiencing significant challenges because of their vision loss, we can help – whether it's your client, patient or someone else you've come across through your work. We count on people like you to refer on behalf of individuals who may not be able to ask for our help themselves.

We have developed this referral form specifically for nurses, personal support workers, medical doctors (non-vision-related), nurse practitioners and Allied Health professionals (i.e. occupational therapists, physical therapists, social workers, discharge coordinators, etc).
