Committing to your rehabilitation care journey

Vision Loss Rehabilitation Canada is dedicated to upholding your rights and providing high-quality rehabilitation therapy. Our goal is to help you regain independence, safety, and mobility after experiencing vision loss.

We want to assure that you, as a client, understand your rights and responsibilities in receiving VLRC services. Similarly, all VLRC staff members are expected to understand and treat each client with the proper respect.

As a client of Vision Loss Rehabilitation Canada, you have the right to:

  • Be treated with respect and dignity regardless of your race, gender, sexual orientation, gender identity, disability, financial status, or ancestry that promotes your individuality and privacy.
  • Receive your service without fear of abuse, discrimination, humiliation, exploitation, harassment, retaliation, or neglect.
  • Receive comprehensive information, intake assessment and service options in a timely and appropriate manner to make informed choices regarding your services.
  • Include other individuals in your home to be present during service when these individuals have been identified in advance.
  • Have a representative such as a substitute decision maker (SDM) or Power of Attorney (POA) for Personal Care who will act on your behalf.
  • To have access to assistance to represent you including, receive information on how to contact a provincial health advocate, to obtain a review of their services or treatment decisions received.
  • Work with Vision Loss Rehabilitation Canada certified Specialists to develop a personalized rehabilitation treatment plan based upon your goals.
  • Receive information and support to access specialized programs and services to address your needs, provided by qualified Specialists.
  • Receive lesson materials and client record information you request in the communication method and format you prefer.
  • Provide your informed consent to the treatment plan and services to meet your needs and can limit, refuse, or withdraw consent at any time. This consent applies to service delivery, release of information, accessing concurrent services, and who is on your service team.
  • Be informed of the nature of treatment and services, their expected benefits, as well as of any risks for participating in services or risks for not having services.

We also ask you to adhere to the following important responsibilities and requirements:

  • Treat Vision Loss Rehabilitation Canada staff, property, and equipment with respect.
  • Prepare for and participate in your therapeutic appointments in a location free from distractions, appropriately clothed with all necessary equipment for the lesson.
  • Provide a service location free of any health and safety hazard, including animals/pets, which may be detrimental to the staff.
  • Ensure that smoking of cigarettes, e-cigarettes, cannabis, or usage of any cannabis-related products will not be permitted one hour before or while staff are present to provide VLRC services.
  • Understand and respect that staff are not permitted to administer medications, provide their personal contact information, or accept money or gifts.
  • Contact staff as soon as possible if you need to cancel or reschedule an appointment; 24 hours' notice is requested.
  • Notify Vision Loss Rehabilitation Canada of any changes to your personal information.

Before or upon admission:

When you become a client of Vision Loss Rehabilitation Canada, you'll receive a welcome package that includes a letter explaining all your rights and responsibilities, along with a helpful service brochure. These materials will be tailored to your province and presented in a language and format that you and your representatives understand best.
